We discover your needs and wishes. We define the right team to help you "solve" and simplify your caregiving journey. All through this process, we collaborate closely with everyone on the team to determine everyone's role and next steps.
Collecting the Hard Data
A key component in establishing the right home or community care is working within your personal, budgetary, and financial needs. Our expert team works to identify needs and resources along with explaining the financial tools available that can help you find a way to PAY FOR CARE.
Needs and Situational ID
We take inventory of your caregiving, emotional and personal needs. We'll review past evaluations, the ADLs, IADLs, family dynamics, and even community components to create an actionable plan (including coaching) that works for your family AND for your aging loved one.
We Deliver, IMMEDIATELY
One of the most critical parts of the process is expedited execution, especially for home care. We take cost, care, assistance needs, safety, and more into account when making sure to deliver on our promises to fix it NOW.
Prep for the Long Game
We take everything we've done and make it accessible and easy to digest via Astrid, our collaborative 24/7 care management technology platform. Caregivers and families use the app to guide treatment decisions, ease the process of caregiving, and make tools available for collaboration that improves outcomes.
We'll also introduce our proprietary coaching support program focused on how to both give and receive care. Coaching modules help caregivers stay well emotionally, manage responsibilities, and find balance as a caregiver.
